How should I be sleeping when pregnant?

Getting good sleep during pregnancy can feel like chasing a moving target. Just when you find a position that works, your growing belly, an achy back, or a 2 a.m. bathroom trip throws everything off again. The good news: while pregnancy sleep is rarely perfect, there’s a lot you can do to make it more comfortable and more restful — starting with how you position your body at night.

If you ask any OB-GYN what the best sleeping position during pregnancy is, you’ll almost always get the same answer: side sleeping, ideally on your left side.

Sleeping on your left side is considered the gold standard because it keeps your growing uterus off the inferior vena cava — the large vein that runs along the right side of your spine and carries blood back to your heart. Left-side sleeping:

Maximizes blood flow and nutrients to the placenta and baby

Improves kidney function, helping your body clear waste and reduce swelling

Eases pressure on your liver, which sits on the right side of your abdomen

Can help reduce swelling in your legs, ankles, and feet

That said, don’t panic if you wake up on your right side or flat on your back — right-side sleeping is still considered safe and reasonable, especially if left-side sleeping isn’t sustainable for a full night. Some newer research even suggests that sleep position earlier in pregnancy (before 30 weeks) may matter less than once thought. Most doctors agree the real priority is simply avoiding long stretches flat on your back, especially in the second half of pregnancy, and getting sleep however you can.

Pro tip: You’re not expected to hold one position for eight straight hours. Shifting positions throughout the night is completely normal — if you wake up on your back, just roll back onto your side and go back to sleep.

Why You Should Avoid Sleeping on Your Back (Later On)

Sleeping flat on your back is generally fine in the first trimester. But as your uterus grows — usually from around 20 weeks onward — back sleeping can put pressure on major blood vessels, potentially reducing circulation to you and your baby. It’s also linked to backaches, digestive discomfort, hemorrhoids, and low blood pressure, which can leave you feeling dizzy or short of breath.

If you’re a lifelong back sleeper, prop a pillow behind you so you naturally roll toward your side, or invest in a wedge pillow to prevent yourself from flattening out during the night.

What About Stomach Sleeping?

Stomach sleeping is generally safe in early pregnancy, and many women naturally phase out of it as their belly grows and it simply becomes uncomfortable. There’s no need to force yourself off your stomach in the first trimester if that’s what helps you sleep — just know it likely won’t stay comfortable for long.

Best Pillow Setup for Pregnancy Sleep

A few strategic pillows can make side sleeping much more comfortable:

Between your knees – Keeps your hips aligned and takes pressure off your lower back.

Under your belly – Provides support as your bump grows, especially in the second and third trimesters.

Behind your back – Helps prevent rolling onto your back overnight.

A wedge or extra pillow under your upper body – Elevates you slightly if you’re dealing with heartburn or shortness of breath.

Many pregnant women find a full-body or “C-shaped” pregnancy pillow eliminates the need to juggle several separate pillows.

How Many Hours of Sleep Do You Need While Pregnant?

Experts generally recommend 8 to 10 hours of sleep per night during pregnancy — a bit more than the 7-9 hours recommended for the general adult population. Your body is doing an enormous amount of work, and adequate rest supports both your health and your baby’s development.

In reality, hitting that number isn’t always realistic, especially in the third trimester. If nighttime sleep is broken up, short daytime naps can help fill the gap.

Managing Pregnancy Insomnia by Trimester

Sleep struggles show up differently depending on the stage of pregnancy:

First trimester: Rising progesterone can make you unusually sleepy, while nausea, frequent urination, and tender breasts can interrupt rest.

Second trimester: Often the easiest stretch for sleep, though vivid dreams, mild back pain, and leg cramps can start creeping in.

Third trimester: The toughest stage for most people. Fetal movement, heartburn, restless legs, shortness of breath, and constant bathroom trips are common — up to 80% of women report insomnia symptoms by this point.

A few things that genuinely help:

Limit fluids in the evening (but stay well-hydrated during the day) to cut down on nighttime bathroom trips

Avoid caffeine in the afternoon and evening

Build a wind-down routine — a warm shower, light stretching, or reading can signal to your body it’s time to sleep

Keep your room cool and dark, since pregnancy often raises your body temperature

Try light exercise during the day, like a walk or prenatal yoga, to help you fall asleep faster at night

Elevate your upper body slightly if heartburn is keeping you up

Talk to your doctor before taking any sleep aid, including melatonin — your body already increases melatonin naturally during pregnancy, and supplementing can interfere with your baby’s developing sleep-wake cycle

When to Call Your Doctor

Occasional restless nights are normal, but mention the following to your provider:

Loud snoring or gasping for air during sleep (possible signs of sleep apnea)

Severe or persistent restless legs

Sudden, unusual shortness of breath

Sleep so disrupted it’s affecting your daily functioning or mood
